Eight statewide ballot questions were on the November 2, 2004 ballot in the state of Nevada.

Key facts
- Through 2002, a total of 41 citizen-initiated ballot propositions had appeared on Nevada's ballot since the beginning of the process in 1918. Of those, 26 passed and 15 failed.
- With six initiatives on the 2004 ballot, the historical total of Nevada citizen initiatives increased to 47. Of the citizen initiatives on the 2004 ballot, three passed and three failed.
